摘要

本文基于系统文献回顾与多理论整合方法，构建产后抑郁（postpartum depression, PPD）多维防治的理论框架体系，涵盖临床筛查诊疗、家庭社会干预以及医护人才教育，为降低围产期孕产妇抑郁发病率并且为重构医疗防线提供思路。本文将基于现代公共卫生与临床医学的视角，梳理PPD的“早筛早诊早治”策略，包括分析爱丁堡产后抑郁量表（EPDS）等早期临床筛查手段、精神药物及心理干预的多学科临床治疗路径。此外，引入“信息-动机-行为技巧”理论构建家庭健康干预模型，并基于社会生态学模型提出了破除疾病污名化的社会支持策略。最后，探讨了创伤知情护理在产后抑郁多维防治中的作用。面对产后抑郁防治体系的不足，本文构建“规范化筛查、前沿医疗治疗、家庭社会赋能、医护教育”的多维体系。

Abstract

Based on a systematic literature review and multi-theory integration approach, this paper constructs a theoretical framework for the multi-dimensional prevention and treatment of postpartum depression (PPD). Covering clinical screening, diagnosis and treatment, family and social intervention, and education for medical and nursing professionals, the framework aims to provide insights for reducing the incidence of depression among perinatal women and reconstructing the medical defense line. From the perspective of modern public health and clinical medicine, this paper systematically sorts out the "early screening, early diagnosis and early treatment" strategy for PPD, including an analysis of early clinical screening tools represented by the Edinburgh Postnatal Depression Scale (EPDS), as well as multidisciplinary clinical treatment pathways combining psychotropic medications and psychological intervention. In addition, this paper introduces the Information-Motivation-Behavioral Skills (IMB) Model to develop a family health intervention model, and proposes social support strategies to eliminate disease stigma based on the Social Ecological Model (SEM). Finally, it discusses the role of Trauma-Informed Care (TIC) in the multi-dimensional prevention and treatment of PPD. In response to the shortcomings of the existing PPD prevention and treatment system, this paper establishes a multi-dimensional system centered on four core dimensions: standardized screening, cutting-edge medical treatment, family and social empowerment, and medical and nursing education.
