The impact of protective isolation measures and standard ward management on infection rates in CAR-T patients
保护性隔离措施与标准病房管理对CAR-T患者治疗后感染率的影响研究

摘要

目的 探讨血液肿瘤科保护性隔离措施与标准病房管理对CART细胞治疗患者治疗后感染率及护理满意度的影响，为临床护理管理提供依据。方法 选取本院和协和医院收治的100例接受CART治疗的患者，随机分为试验组（采用保护性隔离措施，n=50）和对照组（采用标准病房管理，n=50）。比较两组患者治疗后导管相关血流感染、局部感染、静脉炎的发生率及总感染发生率，同时评估护理满意度。结果 试验组患者导管相关血流感染、局部感染、静脉炎的发生率分别为2.00%，2.00%，2.00%，总感染发生率为6.00%；对照组分别为6.00%，10.00%，6.00%，总感染发生率为22.00%。两组总感染发生率差异具有统计学意义（χ²=5.316，P=0.023）。护理满意度方面，试验组总满意度为98.00%，对照组为78.00%，差异显著（χ²=9.470，P=0.003）。结论 血液肿瘤科采用保护性隔离措施可显著降低CART治疗患者的感染发生率，提高护理满意度，建议在临床推广应用以保障患者安全与治疗效果。

Abstract

Objective To investigate the impact of protective isolation measures and standard ward management on post-treatment infection rates and nursing satisfaction in patients undergoing CAR-T cell therapy, providing evidence for clinical nursing management.
Methods A total of 100 patients who received CAR-T treatment at our hospital and UNION HOSPITAL were randomly divided into two groups, with 50 patients in each group. The experimental group(n=50) adopted protective isolation measures, while the control group(n=50) received standard ward management. The incidence of catheter-related bloodstream infections, local infections, phlebitis, and overall infection rates were compared between the two groups, along with nursing satisfaction.
Results The incidence of catheter-related bloodstream infections, local infections, and phlebitis in the experimental group were 2.00%,2.00%, and 2.00% respectively, with an overall infection rate of 6.00%.In contrast, the control group had incidences of 6.00%,10.00%,and 6.00%,respectively,with an overall infection rate of 22.00%.The difference in overall infection rates between the two groups was statistically significant(χ²=5.316,P=0.023). Regarding nursing satisfaction, the experimental group had a total satisfaction rate of 98.00%, compared to 78.00%in the control group, with a significant difference(χ²=9.470,P=0.003).
Conclusion   Protective isolation measures in hematology oncology departments can significantly reduce the infection rates in CAR-T treated patients and improve nursing satisfaction. It is recommended to promote these measures in clinical practice to ensure patient safety and treatment efficacy.
