Application of standardized health education pathway in holistic nursing service for diabetic patients
糖尿病患者标准化健康教育路径在整体护理服务中的应用

摘要

随着社会的不断发展，医学模式的不断变化，人群以及各层面对于相应的护理服务提出了新的要求。现阶段为患者所提供的护理服务以从传统的以患者为中心责任制过度为以人为中心的相应的护理模式[1]。基于整体护理是指护士对患者从新入院、在院接受护理服务的过程中，以及患者出院前后的整个过程中，为患者提供完善的、全程的、个体化的护理服务。由于护士专业知识的广度、深度自我学习能力存在一定的局限性，同时在相关护理人员接纳新知识的速率以及各项综合能力之间存在一定的差异性，使得现阶段院内各患者所接受的护理措施的质量存在不同程度的差异性，其中以入职时间相对较短的护理人员最为突出，相比与入职时间相对较长的护理人员，其在工作的过程中存在不同程度的紧张等方面的心理因素，使其在实际工作的过程中容易出现相应的差错，如在对患者进行心理引导的过程中存在：语言语气不恰当，心理引导的方式不成熟，难以达到预期的效果；在对患者进行健康宣教的过程中，不能有效的抓住宣教内容的关键点，甚至是出现重要宣教内容忽略的情况，进而使得患者对于在整个护理期间的注意事项、禁忌事项等内容的了解不够充分，导致患者的恢复速度以及恢复质量受到不同程度的影响。2016年7月至 2016年12月保定市第一医院在工作工作中融入了整体护理模式的核心内容开展相应的护理工作，为糖尿病患者制定出专业的、全面的健康教育路径[3]，并通过胰岛素控制血糖患者低血糖发生率和提相关满意度等指标了解路径实施效果现介绍相关经验如下

Abstract

In recent years, with the transformation of medical model, the connotation of nursing service has been expanding. Nursing work has been gradually transformed from the patient-centered responsibility nursing model to the human-centered holistic nursing model[1]. Holistic nursing refers to the implementation of comprehensive, full and individualized nursing services by nurses for patients during the whole process of new admission, hospitalization and discharge. Because the nurses professional knowledge of the limitations of breadth, depth, self learning ability and accept of the fast speed of knowledge work ability difference, such as quality of health education on patients will be uneven, especially low qualification nurses, because of the relatively insufficient clinical experience, prone to missing the related content in the health education to explain the problem such as unprofessional. In order to ensure that every patient can receive comprehensive and professional health education during the whole hospitalization process, Baoding First Hospital constructed a standardized health education path for diabetic patients from July 2016 to December 2016 in combination with the overall nursing work of responsibility system [3]. And through the incidence of hypoglycemia in patients with insulin control of blood glucose and patients' satisfaction rate and other indicators to understand the implementation effect of the path, the relevant experience is introduced as follows
